PLEASE PROVIDE THE FOLLOWING DOCUMENTS:
  • Official Birth Certificate (government issued only, not from hospital); foreign certificates must be translated
  • Parent/Guardian’s Current California Driver’s License/Identification Card
  • Immunization Record. Must include the record of Tuberculosis Skin Test
  • Students enrolling from outside of Santa Clara County will be required to have their healthcare provider complete the Santa Clara County Public Health Department Risk Assessment for School Entry Form. TB Assessment form is located under the Forms section on the top, left side of this page. 
  • Verification of withdrawal from previous school
  • Transcript with 2nd Semester Grades/Credits
  • Conditional Documentation: Caregiver’s Affidavit (If Applicable)
  • Current IEP or 504 must be submitted at the time of registration (If Applicable)
CHOOSE WHICH APPLIES TO YOUR HOUSEHOLD AND SUBMIT THE APPROPRIATE DOCUMENTS: 
1. If parent/guardian owns the house, provide the following:
  • Property Tax Bill or Escrow paperwork, PG&E, Water Bill
  • 3 Acceptable mailed communications (see below) 
2. If parent/guardian rents the entire house, provide the following:
  • Current Legal Lease/Rental Agreement in parent/guardians name AND 
  • 3 Acceptable mailed communications (see below) 
3. If parent/guardian rents a room or multiple rooms of the house, provide the following:
  • Lease/Rental Agreement in Parent/Legal Guardians name OR
  • Complete the Residency Form both side - Residency Status Form
  • 3 Acceptable mailed communications (see below)
